Responsibilities:
- Admissions
- Recertification of patients
- PRN Visits if needed
- Patient assessments
- Inform the patient of instructions for medication prescribed by a physician.
- Take and asses vital signs, record patient information, and report changes to the team of physicians.
- Provide emotional support to patients and their families.
- Provide simple wound care if needed.
- Maintain patient records in accordance with established standards.
- PRN visits if necessary.
- Medication reconciliation.
- Checking the overall well-being of patients and any care they may require.
